Establish an Account
You can register online to begin the Account Establishment process. Below are additional forms that will need to be filled out depending on the type of account you would like to open.
To open an account with Credit Card terms:

If you are exempt from sales tax, complete the Tax Exemption Certificate and sign.

Once you are registered, you can enter your credit card account into our website, or call Customer Service to add it for you.
To open an account with Net terms:

Complete ALL sections of the Credit Application and sign.

If you are exempt from sales tax, complete the Tax Exemption Certificate and sign.

Fax these forms to 770-720-1722. You will be notified regarding your account number and the terms of your account.

To inquire about the status of your application, call 800-414-5650.
To change an existing account to Net terms:

Complete ALL sections of the Credit Application and sign.

Fax the completed form to 770-720-1722. You will be notified regarding the terms of your account.

To open an account with COD/Certified Funds terms:
- Call our customer service department at 800-414-5650
To open an account with COD/Company Check terms:
- Complete the Credit Application up to and including the Banking Information section and sign.
- If you are exempt from sales tax, complete the Tax Exemption Certificate and sign.
- Fax these forms to 770-720-1722. You will notified of your account number
- To inquire about the status of your application, call 800-414-5650.
